It is a bit curious, but the most difficult element of this book to write may be this
acknowledgment. Not because I have so few to thank, but because I have so many.
The number of family, friends, mentors and colleagues who have unselfishly shared
their wit and wisdom with me over many years is humbling. I have been, and remain,
